About The Role
As a Senior Personal Carer, you'll conduct initial client intake and screening assessments, develop and review individualised care plans with a client-centred approach, while providing quality care based on clients' needs.
Key Responsibilities Include
- Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.
- Manage client information in CRM & CMS ensuring all details and case notes are kept up to date.
- Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team and as required.
- Spend your time on the road completing scheduled visits, interacting one-on-one with clients within their homes supporting with daily activities.
- Provide care in accordance with their individualised Care Plan.
- Support with community based recreational and social activities
- Transportation and assistance with shopping and appointments.

Role Requirements
- Respite Centre Experience - Highly Regarded
- Certificate III Aged Care, Individual Support or an equivalent qualification - Essential
- Current Australian Driver's Licence (Green Provisional minimum) - Essential (International Licences will not be considered)
- Roadworthy & insured (Third Party Property minimum) vehicle to use for work- Essential
- Experience with technology incl. smartphones, tablets, laptops etc
- Confidence to satisfy the relevant probity checks required by legislation or policies
PLEASE NOTE: This role will be based on the mainland (Alexandra Hills) however there will be a requirement to cover Macleay & Russell Islands (and surrounding areas).
