About the role:
Create special, rewarding relationships by providing regular support and living assistance to those in need within the community!
Our Home and Community Service is seeking a dedicated Home Care Worker to join our team. In this role, you will offer essential assistance to clients, aiding them in daily tasks to maintain independence and comfort at home. Your responsibilities will include personal care, medication assistance, transportation, domestic duties, and assisting clients to meet social needs.
Responsibilities or duties
- Performs a range of activities of daily living which may include but is not limited to hygiene, dressing and grooming, shopping, transport and respite in line with the prescribed care plan.
- Completes domestic duties of a routine nature within the client's home to assist in the provision of a clean, comfortable, safe and secure environment which may include cooking and preparing meals.
- Supports clients to meet their social needs.
- Follows work plan/schedule and prioritise key tasks.
- Assists in administering medication within the scope of the role and as deemed competent.
- Produces accurate written information using forms, log books and templates appropriate to the task.
Qualifications or requirements (e.g., education, skills)
About you:
- Demonstrated ability to work effectively as a member of a team and contribute to a positive, client-focused culture.
- Well-developed interpersonal skills, including the ability to work effectively and sensitively with a range of clients, including those with challenging behaviours, or clients from culturally and linguistically diverse backgrounds.
- Ability to organise and prioritise own work activities and schedules to meet established deadlines and achieve high quality work outcomes.
- Sound verbal and written communication abilities, including the ability to understand instructions, read and navigate road maps, comprehend care plans and communicate clearly and professionally with both clients and staff.
- Sound computer skills.
Role requirements:
- Current National Police Certificate
- Current Drivers Licence and own reliable vehicle
- Current First Aid and CPR Certification
- Certificate III in Individual Support, Aged Care, Home and Community Care, Disability Services or equivalent
