The Opportunity
At Amplar Health, we are in an exciting phase of growth as we continue to expand our Support at Home and Acute Hospital substitution services in Toowoomba creating more opportunities for our clinicians to innovate, develop, and be part of transforming the way care is delivered in the community. We are looking for a Physiotherapist to join our team in a permanent full or part time capacity.
As a mobile clinician, you will be providing care to Australians in the comfort of their home. This is a great opportunity for you to have variety in your caseload which will allow you to utilise your skills and grow as a health professional with continued multidisciplinary clinical support, ongoing professional development and education and mentoring from our senior clinicians.
Responsibilities or duties
In the community you will support clients and programs including:
- Support at Home clients: supporting clients to remain independent at home, through home assessments; equipment prescriptions and home modifications
- Rehab in the Home program: focusing on post orthopaedic surgery and reconditioning
- Hospital in the Home program: supporting clients after hospitalisation, including general surgeries and infection
- Transitional Care Program: supporting clients to return to their base line or new functional capacity and remain safe at home post extended hospitalisation
- After Hospital Home Care: working with a multidisciplinary team to prevent avoidable hospital readmissions
For Residential Aged Care clients, you will:
- Complete assessments, including initials, reviews, post hospitalisation and falls
- Develop, implement, and manage programs
- Design and facilitate group programs
- Complete individual sessions with residents
- Facilitate manual handling
- Prescribe equipment
Qualifications or requirements
About you
New graduate or experienced clinician, you are driven to make a genuine impact in the lives of Australians. With current AHPRA registration, a driver’s licence and access to your own vehicle, you’re comfortable working in a mobile role and travelling across Toowoomba to support clients (kilometre reimbursement provided).
