Your Opportunity

We’re looking for a passionate and skilled Case Manager to join our Families Together Program, delivering intensive, strengths-based support to families experiencing complex challenges.

This is a rewarding opportunity to work alongside families to keep children safe at home, prevent escalation into the child protection system, and create meaningful, long-term change.

You’ll provide holistic, in-home support to families, working flexibly and collaboratively to address a range of needs including parenting, family relationships, safety, wellbeing, and connection to services.

With a pool car provided for outreach work during business hours, you'll have the opportunity to engage directly with families in their homes and communities, providing personalized support where it matters most.

Your key responsibilities will be to:

  • Deliver intensive case management to families with complex needs
  • Conduct home visits and provide outreach-based support
  • Undertake comprehensive assessments and develop tailored case plans
  • Support parents to strengthen parenting capacity and family functioning
  • Identify and respond to risks relating to child safety and wellbeing
  • Coordinate with internal and external services to deliver integrated support
  • Maintain accurate case notes, reports, and compliance documentation
  • Work flexibly, including occasional after-hours support

Requirements for Success

  • Qualification in Social Work, Psychology, Welfare, Community Services or similar
  • Demonstrated experience in case management, particularly with families and children, is essential.
  • Knowledge of issues like domestic violence, mental health, and child development is highly valued.
  • Ability to engage hard-to-reach families and build trust
  • Excellent communication, documentation and stakeholder management skills
  • Current driver’s licence and willingness to travel
  • Current Driver’s Licence.
  • First Aid Certificate.

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
