About the Role

As a Care Planner in our Welcome Team, you’ll play a vital role in setting up new Members and their families for success.

You’ll ensure every detail is covered during onboarding, creating a smooth and positive experience for our Members.

Your key responsibilities will include:

  • Developing and reviewing care plans for new Members
  • Liaising with allied health professionals and external providers to build strong relationships
  • Coordinating with internal teams (workforce, finance, care management) for seamless transitions
  • Managing questions or concerns with professionalism and empathy
  • Planning care within funding budgets and applying for supplements where needed
  • Arranging allied health services to ensure safety and well-being
  • Supporting Members on their journey into long-term care with Five Good Friends

What We’re Looking For

  • Tertiary qualifications in health (e.g., Nursing, Allied Health) desirable
  • Previous experience in care planning or case management
  • Knowledge of My Aged Care, Support At Home, CHSP, DVA, and/or NDIS
  • Understanding of NDIS Practice Standards and Aged Care Quality Standards
  • Ability to work independently and juggle multiple priorities
  • A collaborative team player who shares our values and mission
  • Unlimited working rights in Australia and a successful National Police Check
