About the Role
As a Care Planner in our Welcome Team, you’ll play a vital role in setting up new Members and their families for success.
You’ll ensure every detail is covered during onboarding, creating a smooth and positive experience for our Members.
Your key responsibilities will include:
- Developing and reviewing care plans for new Members
- Liaising with allied health professionals and external providers to build strong relationships
- Coordinating with internal teams (workforce, finance, care management) for seamless transitions
- Managing questions or concerns with professionalism and empathy
- Planning care within funding budgets and applying for supplements where needed
- Arranging allied health services to ensure safety and well-being
- Supporting Members on their journey into long-term care with Five Good Friends
What We’re Looking For
- Tertiary qualifications in health (e.g., Nursing, Allied Health) desirable
- Previous experience in care planning or case management
- Knowledge of My Aged Care, Support At Home, CHSP, DVA, and/or NDIS
- Understanding of NDIS Practice Standards and Aged Care Quality Standards
- Ability to work independently and juggle multiple priorities
- A collaborative team player who shares our values and mission
- Unlimited working rights in Australia and a successful National Police Check
