Based in West Ryde with some local travel required during work hours in the Greater Sydney area

The purpose of this role is to drive a whole of service system approach to deliver person centred outcomes in the areas that Link Wentworth operate. Using a case management framework, you will assess tenants’ support needs, strengths, and support networks to determine their goals, develop case plans, and refer to services to sustain tenancies and increase tenant health and wellbeing.

Key areas of responsibility include:

• Identifying, assessing, and responding to risks that may impact tenant safety, wellbeing, or the sustainability of their tenancy, including risks to the organisation. This includes escalating concerns appropriately, developing risk management strategies in collaboration with tenants and support services, and ensuring timely documentation in line with organisational policies.
• Working respectfully and effectively with tenants from culturally and linguistically diverse (CALD) backgrounds, ensuring services are delivered in a culturally safe, inclusive, and accessible manner, including the use of interpreters where required.
• Supporting tenants experiencing hoarding and squalor by undertaking sensitive assessments, coordinating appropriate interventions, and working collaboratively with specialist hoarding services to mitigate risks and promote safe living environments.
• Working with tenants who present with substance use challenges by applying a trauma-informed, harm‑minimisation approach and linking tenants to appropriate AOD (alcohol and other drugs) services to stabilise and sustain their tenancy.
• Supporting tenants with mental health complexities, including recognising indicators of mental health distress, engaging relevant mental health providers, and ensuring appropriate escalation pathways are followed to maintain tenant and community safety.
• Conducting psychosocial assessments of tenants’ needs to develop responses to address tenancy issues and underlying issues that may affect their tenancy.

Who we are looking for:

The ideal candidate will:

• Have a tertiary or vocational qualification in community services, case management, social work, or similar discipline, or equivalent relevant social services experience
• Demonstrated experience or understanding of the social housing sector and the customers who seek or live in social housing
• Experience in the development and implementation of case plans and working with highly vulnerable people with complex needs and challenging circumstances
• Demonstrated ability to maintain cultural competence in delivering services
• Good negotiation and conflict resolution skills, with the ability to liaise with a number of stakeholders including tenants from diverse backgrounds
• Ability to develop and maintain effective partnerships with support agencies and community services
• Empathy and compassion for others who face disadvantage

You will also need:

• A valid National Police Check (less than six (6) months old) to the satisfaction of Link Wentworth, to be renewed every five (5) years or more frequently as reasonably required by Link Wentworth
• Willingness to comply with vaccination requirements imposed by Link Wentworth, including COVID-19 and other vaccinations, as determined by the organisation in accordance with current Government health advice, mandates, or directions
• Willingness to travel between Link Wentworth offices in metropolitan Sydney
• You may be required to work from a Link Wentworth office up to five (5) days per week. Specific office attendance requirements will be determined by your manager in line with business needs and the requirements of your role. Please note: during your probationary period, office attendance expectations may differ.
• Access to a registered and roadworthy vehicle
• A current driver’s licence valid in the state of NSW
