Pathways Program Facilitator

Are you passionate about supporting young adults to build confidence, independence and real-world life skills? We’re looking for Pathways Program Facilitators to join our team and support the delivery of our Pathways to Independence Program. This is a hands-on role suited to someone who enjoys facilitating group activities, building rapport with young adults, and supporting young NDIS participants to explore their next steps towards independence.

What is the Pathways to Independence Program?

Pathways to Independence is a structured, centre-based group program designed to support young-adult participants to build confidence, independence and practical life skills aligned with their NDIS goals. The program focuses on areas such as:

  • Communication and social skills
  • Emotional regulation and self-awareness
  • Daily living skills and routine-building
  • Confidence, independence and community readiness

Pathways is delivered through planned workshops and activities, supported by facilitators and coordinators who guide sessions, encourage participation, and adapt activities to individual needs. As a Pathways Program Facilitator, you’ll support participants during sessions, help them engage in activities, practise skills, and feel safe, included and supported.

Key Responsibilities

  • Support and facilitate daily group workshops and structured activities
  • Provide hands-on participant support, adapting activities where required
  • Encourage engagement, confidence-building and peer interaction
  • Support routines, transitions and group participation
  • Assist with session set-up, pack-down and resource preparation
  • Complete attendance records, session notes and documentation
  • Report incidents, hazards or participant concerns as required
  • Work collaboratively with Coordinators and Support Workers
  • Follow NDIS Practice Standards, duty of care and safeguarding requirements

About You

You’re someone who:

  • Enjoys working with young people and facilitating group activities
  • Communicates clearly, calmly and respectfully
  • Can follow structured programs while remaining flexible to participant needs
  • Understands the importance of documentation, safety and compliance
  • Works well as part of a team and takes on-shift direction

Essential Skills & Experience

  • Experience supporting people with disability and/or psychosocial support needs
  • Experience facilitating or supporting group-based activities
  • Strong interpersonal and communication skills
  • Ability to complete session notes and follow NDIS requirements

Mandatory Requirements

  • NDIS Worker Screening Check
  • Working with Children Check (if applicable)
  • First Aid & CPR (or willingness to obtain)
