Description

The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West, Great South, Upper Wheatbelt and Peel regions of Western Australia. SWAMS delivers a wide range of Services including GP Clinics, Primary Health Care, Counselling, National Disability Insurance Scheme (NDIS), and community host various programs. SWAMS has a strong growth strategy through partnership opportunities and future community development.

SWAMS is dedicated to enhancing employment opportunities and outcomes for Aboriginal and Torres Strait Islander people. About the role

The Remote Community Connector (RCC) plays a vital role in supporting Aboriginal and Torres Strait Islander people to understand and access the NDIS. This position focuses on building trust, raising awareness, and providing culturally appropriate information to individuals, families, and carers about disability supports available through the NDIS.

Working closely with community members, local organisations, and service providers, the Connector helps identify eligible participants, supports them through the application process, and ensures they feel confident navigating the system. This role is ideal for someone who is community-minded, culturally aware, and passionate about improving access to disability services in remote areas.

We are seeking two Remote Community Connectors – one to work in the Katanning area and one to work in the Narrogin area.

Selection Criteria:

  • Demonstrated experience in advocating and supporting others to reach goals or outcomes.
  • Ability to prioritise tasks and follow up, particularly in busy environments.
  • Able to communicate clearly and confidently in both writing and conversation, using persuasive communication to influence, inform and build understanding.
  • Strong understanding of, and experience working in disability services, in particular NDIS services. Awareness of disability-related challenges and a willingness to learn about NDIS supports, with any lived or community experience considered valuable.
  • Demonstrated ability to build trust and rapport with individuals and groups to achieve positive outcomes for them.
