We're now recruiting a Co-Care Partner to join our Sydney team and help take PlanCare’s people experience to the next level.

This role is full-time, Mon–Fri, 9 AM to 5 PM based in our Parramatta office.

Client Onboarding, Assessment & Care Planning

  • Welcome new clients and guide them through a smooth onboarding experience
  • Conduct regular assessments to understand wellbeing, goals, risks, and support needs
  • Explain how Support at Homework works in a clear, supportive way
  • Develop personalised care plans aligned with client goals, preferences and available funding
  • Regularly review and update plans as circumstances or needs change

Client Support, Collaboration & Problem Solving

  • Respond to client and provider enquiries via phone and email
  • Troubleshoot issues regarding services, billing, wellbeing changes or provider performance
  • Identify and escalate at-risk or vulnerable clients
  • Work collaboratively with onboarding, finance, quality, and service delivery teams
  • Share ideas, insights, and feedback to improve our systems and client experience
  • Participate in training, development sessions, and team huddles
  • Contribute to a supportive, positive team culture

Essential

  • Experience in a client-facing role such as case management, care coordination, NDIS/HCP support in a service-based environment.
  • Relevant qualification in Aged Care, Community Services, Individual Support, Nursing, Social Work or similar — or strong practical experience supporting older Australians.
  • Strong understanding of (or willingness to learn) Home Care Packages, Support at Home Program, and community care compliance.
  • Demonstrated ability to support clients with empathy, build trusted relationships, and communicate clearly with clients, families, and providers.
  • Excellent organisation, time management, and attention to detail, especially when managing caseloads, client records, and invoices.
  • Confident using digital tools — including client management systems, MS Office, email, rostering platforms, and cloud-based systems.
  • Strong problem-solving skills with the ability to troubleshoot issues, follow processes, and escalate risks appropriately.
  • Ability to work both independently and collaboratively within a busy, fast-paced team environment.

Highly Desirable

  • Experience working with My Aged Care, Support at Home requirements, Home Care Packages, budgeting and rostering
  • Experience with case management, particularly supporting older people, vulnerable clients, or those with complex needs.
  • Experience using PRODA, MAC Portal, or Services Australia systems.
  • Clinical qualifications such as RN Division 1, EN, or similar (highly valued but not essential).
  • Lived or learned experience of ageing, caring, disability, or navigating care systems.
