The Team:
Renal Services (North) provides CKD education, Home Therapies services (PD & HD), Haemodialysis for Chronic outpatients, and Acute inpatients, as well as Renal transplant workup and education for eligible patients; preparation for going to Melbourne for transplantation and follow-up care for renal transplant recipients and living donors. We have a dedicated team, committed to providing holistic care to patients and families on their journey with Renal Disease.
The Role:
The renal transplant co-ordinator provides clinical leadership, expertise and co-ordination, for Renal Transplantation, in a multi-disciplinary team, located at the Launceston General Hospital and the Launceston Community Health Centre - Satellite Renal Unit - in Kings Meadows.
The renal transplant co-ordinator focuses on education and co-ordination of recipient and donor work-up programs and long term follow-up of these patients.
Primary responsibilities include:
- receiving referrals, co-ordinating education, workup of recipients and donors, organising investigations, clinical reviews and assessment; formulating a plan for transplant and follow-up; training and access to Organ-Match to access tissue-typing, matching and ability to update status on transplant list in real-time.
- Develop and support the delivery of recipient / donor education and staff education.
- Participate in collation of data in ANZDATA, ANZOD Registry and KPIs.
You will need:
The Renal Transplant co-ordinator needs to have a comprehensive understanding of Chronic Kidney Disease, and the benefits of Renal transplantation, for potential recipients.
The Renal Transplant co-ordinator needs exceptional organisational skills, to be able to co-ordinate referrals for transplantation / donor work-ups from local Nephrologists, Interstate transplant centres, and to co-ordinate work-up protocols and assessments according to the transplanting hospital protocols.
The Renal Transplant co-ordinator needs exceptional education skills, to be able to develop and maintain a coaching approach to patient education, providing timely, consistent and appropriate education and support to patients and families undergoing renal transplantation or donation and follow-up / education post donation / transplant.

Details of appointment:
Permanent, part time, day worker position, working up to 40 hours per fortnight, commencing from as soon as you can join us.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.
Salary: $109,767 to $115,813 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.
Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.
Access to the Grade 4 salary range $102,439- $109,175 is subject to successful application for progression to Grade 4
In addition, the following allowances calculated on the salaried incremental point may be available:
- Post Graduate Allowance of up to 10% based on qualification type
Salary Packaging:

- Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse
Applicants should note the following criteria are desirable:
- Holds or is working towards a relevant post graduate qualification
- receiving referrals, co-ordinating education, workup of recipients and donors, organising investigations, clinical reviews and assessment; formulating a plan for transplant and follow-up; training and access to Organ-Match to access tissue-typing, matching and ability to update status on transplant list in real-time.
