About the Role

We’re looking for a Care Specialist to join our Care Team as we continue to grow. Care Specialists are qualified health professionals who support Members with more complex and clinical needs.

Working closely with Care Team Leaders, Care Coordinators, Workforce Coordinators, and Helpers, you’ll help ensure high-quality assessment, planning, and delivery of person-centred care. You’ll also build trusted relationships with Members and their families, providing responsive, proactive clinical guidance and support.

This is a role where your clinical expertise directly improves lives—while being supported by strong systems, teamwork, and technology.

What You’ll Be Doing

  • Responding to remote care monitoring alerts and coordinating follow-up with Members, families, and care teams.
  • Supporting holistic care planning by assessing complex needs, updating Help Plans, and managing budgets alongside Members and Authorised Representatives.
  • Partnering with Helpers and the Care Team to ensure services align with assessed needs and documented care plans.
  • Building strong working relationships with healthcare professionals and external service providers.
  • Using digital tools and data (including remote monitoring technologies and My Aged Care systems) to support safe, evidence-based care.
  • Supporting compliance with Aged Care Quality Standards and contributing to audit and certification readiness.
  • Delivering exceptional service that contributes to strong Member satisfaction and positive Net Promoter Scores.

About You

  • Tertiary qualification in Nursing with current AHPRA registration.
  • Experience assessing, planning, and coordinating complex care in home care or community settings.
  • Strong understanding of the Support at Home program and Aged Care Quality Standards.
  • Confident using digital tools and data to inform care decisions.
  • Comfortable working within multidisciplinary teams.
  • Person-centred, collaborative, and committed to high-quality outcomes.
  • Tech-savvy, adaptable, and proactive in your approach.
  • Passionate about home care and building meaningful relationships.
  • Able to complete a National Police Check and/or hold a current NDIS Worker Screening Check.
